The reason it's called Arkham City is because essentially a section of the city was cornered off and used as a new Arkham. Essentially the idea was to keep the criminals contained and let them kill each other like in Escape from New York.
I think one reason you might be a bit disappointed with this one is because it lacked the surprise of the original. They were probably trying not to fix things that weren't broken so you had stuff like the combat being fairly similar to the last game.
Still, i'll take your thoughts into account when buying it and see if I can make more sense of the story.
